Question Title

1. As you are aware, GDPR legislation came into effect in May 2018. In line with this, we would ask that you read the GDPR Consent Agreement below & select one of the two checkboxes when filling out the form.

This Privacy Notice is issued by the FAI Schools Leinster Branch c/o National Sports Campus, Abbotstown, Dublin 15. It provides an overview of the data relating to you that we may collect and use in connection with the websites www.faischools.ie & www.fainet.ie how we will use it and how you can exercise your rights in relation to your data. What personal data do we collect and process? The personal data we collect includes: your name and contact details, including your home address (registrars only), work address, phone number and email address; information that you provide in any online application form, any other personal data relating to you that you provide to us or that we generate about you in connection with your use of the website www.fainet.ie

Purposes of Processing We will use the data provided for the purposes of:

- informing other schools/mentors for the purposes of arranging matches;
- informing the relevant competition organisers for the seemly organisation of competitions;

- informing the FAI Schools Administrator for the purposes of arranging national competitions; improving and developing this website; - - informing our accredited media partners for the purposes of reporting on FAI Schools matches; engaging in a consultation process; carrying out research and development surveys; sending you promotional and marketing materials, from our accredited partners; generating and analysing statistics regarding FAI Schools activities; (where possible, personal data will be anonymised before being used for this purpose); fraud prevention, investigation and detection; establishing, exercising or defending legal claims; performing our functions as a public authority and complying with our legal obligations.

Your rights: You have the following rights, in certain circumstances and subject to certain restrictions, in relation to your personal data: the right to access your personal data; the right to request the rectification and/or erasure of your personal data; the right to restrict the use of your personal data; the right to object to the processing of your personal data; the right to receive your personal data, which you provided to us, in a structured, commonly used and machine-readable format or to require us to transmit that data to another controller; and where our processing of your personal data is based on you having provided consent, the right to withdraw your consent to the processing at any time by emailing declan.mcgrath@faischools.ie

THE CLOSING DATES FOR RECEIPT OF ENTRIES FOR SEASON 2022-23 ARE;

• Deadline 1: Senior Cups & Leagues only by Wednesday 7/9/23

No entries allowed after these deadlines.

3. Terms & Conditions of Entry & Submission of Accurate Details

TERMS & CONDITIONS OF ENTRY:

1. The accurate completion of the online entry form is the responsibility of the concerned mentor/s only. Mistakes in the information provided (competitions entered etc.) can only be rectified if notified to the Affiliation & Competition Entry Secretary before the issuing of fixtures. Only in instances where the error is at our end will fixtures be amended after they are issued.

2. Payment of fees is NOT a guarantee of entry, as the online entry form & fees payment processes are separate.

3. If you need to make amendments to your submitted entry please contact me ASAP directly by e-mail to declan.mcgrath@faischools.ie with details - PLEASE DO NOT RESUBMIT THE ENTRY FORM.

3. Leinster B Cups - Proof of Eligibility. If you are entering any of the B Cup competitions, you need to provide proof of eligibility. A template letter has been sent by e-mail to assist you in this matter & is available on the website faischools.ie. Letters must be submitted by the following process; a) template letter must be printed onto school headed paper; b) the letter must record accurate numbers of students in the school - no estimates; c) the letter must be signed by the School Principal/Deputy Principal; d) send on or before Friday 15th October 2021 by e-mail only (PDF or JPEG files accepted) to Leinster Branch Secretary Joe Carroll (joe.carroll@faischools.ie) & cc'd to me (declan.mcgrath@delasalle.ie). Failure to prove eligibility by the date above will result in removal from the affected competition/s, with no refund issued.

ACCURATE SUBMISSION OF DETAILS:

1. BEFORE YOU COMMENCE THIS ONLINE ENTRY, PLEASE ENSURE THAT YOU HAVE TO HAND ALL RELEVANT DETAILS.

2. PLEASE ENSURE THAT ALL DETAILS - ESPECIALLY MOBILE NUMBERS & E-MAIL ADDRESSES, BELONG TO THE MENTOR TAKING THE TEAM & ARE 100% ACCURATE.

3. INCOMPLETE FORMS OR FORMS GIVING INACCURATE INFORMATION WILL NOT BE PROCESSED FOR ENTRY.

4. PLEASE ENSURE THAT YOU FULLY COMPLETE THIS FORM - YOU MUST GET A 'THANK YOU' MESSAGE STATING THAT YOU HAVE COMPLETED THE SURVEY, OTHERWISE THE FORM WILL BE RETURNED AS INCOMPLETE & WILL BE DELETED BY THE SYSTEM.

5. IF ANY OF THE INFORMATION CHANGES - CHANGE OF MENTOR, CHANGE OF MOBILE NUMBER, CHANGE OF E-MAIL ETC. - THE HONORARY SECRETARY (declan.mcgrath@faischools.ie) & THE RELEVANT COMPETITION ORGANISER MUST BE NOTIFIED IMMEDIATELY.

6. IF YOU WITHDRAW FROM COMPETITIONS, THE HONORARY SECRETARY (declan.mcgrath@faischools.ie) & THE RELEVANT COMPETITION ORGANISER MUST BE NOTIFIED IMMEDIATELY.

4. Age Qualification Dates

Senior Girls (U-20): born on or after 1/1/2003

Senior Boys (U-19): born on or after 1/1/2004
